Okay, rule number one – treat all cannons as if they are loaded.

That said, if it was a muzzle-loader, I’d imagine it’d be pretty hard to not know it was loaded. The only thought I could have was that they meant to fire a blank load and someone dropped a ball down the barrel.
OR
It was deliberate!
